Individual Support Planning occurs within the framework of the Disability Act 2006. Plans:
• are individualised and directed by the person with a disability;
• consider and respect the role and contribution of the family and significant others;
• emphasise the importance of exploring and identifying informal, generic and funded supports;
• advance inclusion and participation with the aim of fulfilling aspirations, goals and needs of a person with a disability.
EDAR ensures:
• staff are familiar with the guiding principles for planning;
• staff are supported to undertake the planning process with appropriate resources and tools;
• opportunities for people with a disability, families and carers to develop their skills to direct planning to the greatest extent possible.
Key Workers:
• Listen…to what the person with a disability wants;
• Support…the person with a disability to identify their goals and aspirations;
• Commit…to an individual through agreeing on actions and outcomes;
• Advocate…with the individual where necessary.
